This method is a painless assisted reproductive technique applied to patients diagnosed as infertility who cannot become pregnant, despite regular sexual intercourse for one year. During the ovulation period, women receive various stimulant medications (pills or low-dose hormone injections) undergo an 8-12 day preperation period. 35 to 36 hours after the fracture needle, the sperm cells taken from a man are undergo various procedures. What is infertility?

The inability to become pregnant after one year of regular sexual intercourse without contraception (protection) is called infertility. Regular sexual intercourse in this definition, is defined as 2 or 3 times a week on average. If no pregnancy has ever occurred before is called primary infertility. If pregnancy has occurred before or after live birth and if no pregnancy has been achieved after this pregnancy is called secondary infertility. While infertility can be of male and female origin, sometimes unexplained infertility may be seen in 10-15% of parents whose pregnancy cannot be achieved without any reason. What is Invitro Fertilization (IVF) and Microinjection (ICSI)?

In infertile couples; After conducting the necessary examinations and analyzes on the 2nd or 3rd day of the menstrual cycle in women, individual treatment is determined, stimulating hormone preparations are used and follicles be developed in the ovaries. When the follicles reach the desired size, egg collection (OPU) is performed. On the day of egg collection, the sperm obtained from the male and the eggs obtained from the female are combined by microinjection (ICSI) after various processes. Microinjection method; is the process of injecting sperm selected under a microscope into mature eggs with micropipettes attached to the tip of a special microscope. Nowadays, microinjection method which can increase the fertilization rate to 80-90% is used in many in vitro fertilization centers.


In addition to the microinjection method, the IVF method (Invitro Fertilization), which was previously used more widely, but is now less preffered; alongside the collected mature and semi-mature eggs, male's healthy and processed sperms released free It is expected that sperm will enter the egg and fertilize on its own. External intervention is not performed. In fact, IVF treatment is the common name for these two methods.

New applications for increasing the chances of success in IVF treatments are as follows:


Embryo Glue: Eggs and sperm cells, combined under laboratory conditions, are grown in a liquid medium that imitating the natural environment of the mother's uterus. Despite the fact that with the help of high technology, the highest result is achieved,  it is not yet possible to provide one-to-one similarity. The embryo glue technique, which has been used frequently recently, has increased the similarity level of the embryos in the nutrition and living environment. During embryo transfer, embryos taken into this culture medium are transferred to the uterus using a thin catheter. This new technique has been shown to increase the retention rate of embryos in  to the uterus, but there is no study to increase the chance of one-to-one pregnancy success.


Microchip : It was first discovered by a Turkish scientist at Harvard University. Nowadays, the decrease in the number and motility of sperms etc. factors account for 50% of infertility problems. It is medically known that sperm cells with reduced number and motility decrease pregnancy  rates. This method allows to separateion of healthy and high-quality sperm. It was observed that pregnancy rates increased when the procedure was performed with healthy sperms obtained. One of the most important features is the separation of sperm with DNA disorders. 


Budding with Laser: The merging of a sperm and an egg is called an embryo. After embryo formation, an increase and development of the number of cells are observed with each passing day. On the 5th day, the embryo tears out its outer membrane so that it can cling on the mother’s womb but sometimes it cannot come out of the membrane and attachment may not occur. In these and similar situations, In these and similar situations, small sections are opened with advanced laser systems without touching the embryo, creating an area where the embryo will bud. Thus, the embryo can be more easily budded and pulled into the uterus to remove it from the outer membrane. As maternal age increases, this membrane gradually becomes thicker and may reduce the frequency of pregnancy. Repetitive unsuccessful in vitro fertilization is a method that can be applied in cases where the female age is high and the membrane tissue is thick.

AFTERCARE

Insemination is much simpler and cheaper than other assisted reproductive methods. The same precautionary methods that apply to in vitro fertilization with microinjection are used for insemination. An average pregnancy rate of 10-30% is achieved. This rate may vary depending on the person and circumstances.


What should be considered after Insemination?

  • For insemination, there must be an egg that the sperm can fertilize. In other words, ovulation should not be a problem in women.
  • In order for the eggs and sperm cells to meet, it is necessary that the intra uterine and uterine-ovarian passages are open. Before insemination, it shoul be confirmed that the fallopian tubes and uterus are open with radiography film.
  • The results of the sperm analysis of the prospective father should be normal or at least close to normal.
  • There should not be any problems in the tissue covering the uterus (endometrium) that could prevent the fertilization process.

It is necessary to use the prescribed drugs at a dose and within the time prescribed by the doctor, during the treatment period, avoid drinking alcohol and cigarettes, etc., do not take any other drugs without consulting a doctor and come on the day of the procedures with a full bladder.

The treatment process begins with examinations, which will be carried out on the 2nd or 3rd day of the menstrual period. Some patients may only need medications in pill form, and sometimes injections of low doses of hormones may be required. You will be informed separately by your doctor who will perform the operation. Within 8-12 days of treatment, the expectant mother should have an average of 3 or 4 examinations. In the process of examination, the actual process planning shall be made when sufficient response is obtained from the ovaries. In some patients, an excessive number of matured eggs can lead to a cancellation of the procedure, in order to prevent pregnancies of triplets or quadruple. It is not necessary for the prospective father to come to the clinic during the 9-12 days of treatment. It is sufficient to arrive in the morning on the day of the procedure prescribed by the medical staff.
